1 A 22-year-old man is preoccupied with the idea he has stomach cancer. He has had several investigations that rule it out, but he is difficult to reassure. He seems to eventually accept that he does not have this condition and instead insists he has bowel cancer. His mood is low and he is very anxious, but he does not fulfil criteria for a diagnosis of depressive or anxiety disorder.

2 A 33-year-old woman is referred to a psychiatry out-patient clinic. She has had a number of gastrointestinal symptoms for several years, and also aches and pains in her joints. He did not engage in any untoward behaviour during his trip.
